1. Who we are and what Omniform is
Omniform is a self-help document preparation tool operated by Construct Systems LLC. It helps you fill out forms by asking plain-language questions and placing your answers, at your direction, onto the form. Omniform is not a law firm, accounting firm, or tax preparer; it does not provide legal, tax, financial, or immigration advice, does not recommend what you should file or select, and is not affiliated with or endorsed by any government agency. The blank forms themselves are always available free of charge directly from the issuing agency.
2. Your responsibilities
You are the preparer of your documents. You are responsible for the truth, accuracy, and completeness of every answer, for reviewing the completed document before downloading or submitting it (the service requires you to confirm this review), and for any filing requirements, deadlines, signatures, and fees of the receiving agency. For advice about your specific situation, consult a licensed professional.
3. Artificial intelligence
Omniform uses artificial intelligence to understand your messages, author questions for uploaded forms, and organize the conversation. AI output can contain errors. Every value placed on a form is validated against the form's structural rules, but Omniform does not guarantee correctness — your final review is part of the service's design, not a formality.
4. Accounts, uploads, and identity verification
Uploading your own form requires an account and a payment method on file. One free upload is available to accounts that complete identity verification (provided by Stripe). You agree not to upload documents you have no right to use, content that is unlawful, or files intended to disrupt the service. We may decline or remove uploads at our discretion.
5. Fees and refunds
Catalog forms marked free are free. Uploaded forms are priced individually — the price is quoted before processing and you are charged only when you download the completed document. Payments are processed by Stripe; Omniform does not store your card number. People who receive a form through Omniform's send feature fill it at no cost. If a completed document is materially defective due to a fault in the service, contact us within 14 days for a refund of that document's fee; refunds are otherwise at our reasonable discretion.
6. Your data
Your answers belong to you. Our Privacy Policy describes what we collect, how identifiers like SSNs are protected, which third parties process data, and our retention and deletion rules — including your ability to delete a form's data or your entire account at any time.
7. Disclaimers and limitation of liability
The service is provided "as is" without warranties of any kind, express or implied, including fitness for a particular purpose. To the maximum extent permitted by law, Construct Systems LLC's total liability arising out of or relating to the service is limited to the amount you paid for the document giving rise to the claim, and neither party is liable for indirect, incidental, or consequential damages. Nothing in these terms limits liability that cannot be limited by law.
8. Termination, changes, and contact
You may stop using the service and delete your account at any time. We may suspend accounts that violate these terms. We may update these terms; material changes will be posted here with a new date, and continued use after a change constitutes acceptance. These terms are governed by the laws of the state of Construct Systems LLC's organization, without regard to conflict-of-law rules.
